New Patient Information
Please bring the following information with you on your first visit with us...
- Please bring all insurance cards with you so they may be copied for your permanent file.
- For a Worker's Compensation claim or Motor Vehicle claim, bring the correct billing address, phone number, contact person, and claim number for the injury.
- If a referral is necessary, please obtain one from your PRIMARY PHYSICIAN PRIOR to the visit and bring it with you. If you require a referral and do not have it with you at the time of your appointment, your appointment may need to be rescheduled.
- Please bring X-ray or MRI films if there were any studies completed and you were asked to do so when you made the appointment.
- Please be prepared to pay any co-pay due from your insurance according to your policy (sometimes indicated on your insurance card).

Healthcare Insurers
Argires, Becker, Marotti & Westphal participates with most major healthcare insurers. If you are unsure whether we participate with your insurance carrier, please contact our office. We request that your copay be paid at registration, or we will have to reschedule your appointment.
Appointments
Please make an appointment in advance by calling our office at 717-358-0800.
If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery. Please remember to bring your insurance information and a list of your current medications to your appointment.
Argires, Becker, Marotti & Westphal follows all federal and state laws and reporting requirements regarding identity theft. Please be informed that it is a requirement that you bring a photo ID issued by a local, state or government agency (valid driver's license, "green card", passport, employee ID card, or student ID card, current utility bill if photo ID submitted does not resemble patient) to your appointment. If the patient is a minor, the patient's parent/guardian should bring the information listed above.
A Neurosurgeon and Orthopedic Physician is always on call after regular office hours for urgent problems.
